O'Connell Care at Home is expanding and we are looking for a full time, part time or per diem Registered Nurse (RN) to join our team at our BRAND NEW location in Auburn, MA (office located at 325 Southbridge Street will be opening in March 2024.) serving the greater Auburn area!

General Job Description 

The RN provides individualized nursing care, case management and comprehensive services to clients and families in their own home under the supervision of the Director of Nursing. The RN is responsible for a specific caseload of clients, provides on-the-job training and support for caregivers, and is a resource to their designated office as well as key stakeholders.  

Duties and Responsibilities (including but not limited to): 

Assist with bed positioning, standing, walking of patients and provide education and instruction to caregivers of proper techniques.  

Medication management 

Completes documentation timely, accurately, and at the point of care, according to industry standards, conventions and guidelines.  

Schedule, document, and complete field staff supervision visits quarterly on all caregivers. 

Ensure caregivers are following proper procedures, care plan and agency regulations. Provides training as needed.  

Provides assistance and direction to problems encountered by both office staff and field staff. 

Assess the needs of clients and the abilities of field staff to ensure the best possible care for clients. 

Complete new client assessments, care plans, and updates as necessary. 

Assist with new hire orientation and skills checks. 

Coordinate case management of clients in collaboration with Director of Nursing and Office staff. 

Follows company policies and practices as outlined in the Employee Handbook and/or applicable employment agreement. 

Performs additional related duties as assigned. 

Minimum Requirements 

Must possess a current and valid RN license in Massachusetts 

Graduate of an accredited RN program 

Valid CPR Certification

Minimum of two years' experience in the health care field; homecare preferred 

Possess a positive attitude, a willingness to learn, with a desire to be an effective member of the team 

Excellent communication skills and the ability to build meaningful relationships with internal and external stakeholders 

Comfortability using various forms of technology and software (Wellsky) 

Cognitive skills as related to the position. 

Thorough understanding of Private Case vs ASAP Cases and their requirements  

Ability to participate in the on-call rotation. 

Due to the nature of our business or a myriad of other factors, employees may need to work from home. For that reason, access to reliable secure internet service at home is required. 

Reliable transportation, current driver’s license, and ability to drive to our various locations as well as the locations of our clients and stakeholders on a regular basis.  

Must be able to move or lift patients and use proper posture to avoid injury 

Ability to lift fifty (50) pounds.  

Skilled in use of mechanical and Hoyer lifts. 

Ability to stand for extended periods.
